Orthodontics: how do I submit my expenses to Vanbreda?  

Important: you must first submit your expenses to your mutuality to apply for the statutory and supplementary intervention, before submitting your expenses to Vanbreda.  

What documents does Vanbreda then need?   

1. The invoice or fee honorary note from your orthodontist with the following information:  

  • the date of your visit 
  • the nomenclature code of the service or a description if no code exists 
  • the amount you paid  
  • your name and personal details  

2. The proof of reimbursement from your mutuality with statutory and supplementary intervention with the following details: 

  • the date and codes of the services provided and the amount your mutuality reimbursed per nomenclature code from the statutory health insurance. If you do not receive reimbursement, provide us with proof from your mutuality that you do not receive any statutory intervention.  
  • the date and codes of the services provided and the amount the mutuality reimbursed from the supplementary insurance. If you do not (or no longer) receive reimbursement (e.g. because the maximum reimbursement was reached), provide us with proof of this.   
  • Your mutuality sometimes calls this document a ‘kwijtschrift’.  

3. Did you also receive a reimbursement from another additional dental insurance?    

  • Provide us with proof of their intervention per service.
